Data we collect
Through the diagnostic form: name, role, practice name and type, specialty, city, phone, email (optional), preferred time and message. We also record the page you came from and, where present, campaign parameters (UTM).
Purpose and legal basis
This data is used only to contact you to arrange your growth diagnostic and, if you wish, to send you a proposal. Processing is based on the consent you give when submitting the form.
Recipients
The data is accessible to the OutPerform Health team and stored in our internal tool. It is hosted by our technical providers (website hosting, database, email delivery). It is never sold or transferred.
Retention
24 months from our last exchange, unless you become a client (for the duration of the contract, then as required by law).
Your rights
Under Moroccan law no. 09-08 on the protection of individuals with regard to the processing of personal data, you have the right to access, rectify and object. To exercise them: contact@health.outperform.ma.
